Significance of Intraoperative Cholangiography



Abstract
Despite the usefulness of intraoperative cholangiography (IOC), its routine application is controversial. We reviewed 95 patients who underwent surgical treatment for gallbladder stones and had intraoperative cholangiography. Jaundice,
pancreatitis,
operative findings of dilated extrahepatic ducts and palpable common bile duct stones were included in the criteria for indications of selective intraoperative cholangiography. The positive IOC rate in indicated cases was 40.0% and 1.3% in
not-indicated
cases(p<0.05). Overall false-positive rate was 2.1%.
The results of this study indicate that intraoperative cholangiography is safe and accurate for diagnosis of common bile duct stones. The advantages of selective operative cholangiography versus routine cholangiography are discussed in detail. We
concluded that the selective operative cholangiography for patients who were at risk for having common bile duct disease should be the preferred procedure.
